Application & Waivers

Please follow the directions below:

  • Click the link below and download the three page document to your computer.
  • Complete the application and waivers electronically and save it.
  • Print the document and sign on pages 1, 2, & 3 using wet ink where a "hand signature" is indicated.
  • Upload your documents below or email the completed application and waivers to tmcelwain@ocsheriff.gov.
